Once an applicant is hired, they will need to complete a New Hire packet prior to the first day of employment. These forms can be found by clicking the links to the right.
1. Please select the link to the right for the appropriate checklist for each employee being hired.
2. Download and print each form listed on the checklist. All of the forms, except for the Checklists and the Handout Receipt form, are interactive so they may be typed into for convenience.
3. The Background Check form and E.E.O. forms are included in the Employment Application packet. Most forms on this page are interactive and can be completed simply by typing in the appropriate information. The blank forms may be saved to your desktop. Upon completion of the above training courses, please print the certificates of completion, sign them, and return to the Human Resources Office.
